  • Conducting comprehensive intake assessments with patients and families.
  • Ensuring families feel equipped and supported.
  • Developing individualized treatment plans considering physical, financial, and other constraints.
  • Adjusting treatment protocols as needed.
  • Suggesting and coordinating follow-up services to promote wellness.
  • Ensuring patients have transportation to and from their homes and healthcare facilities.
  • Collaborating with healthcare teams to discuss patients' progress and deliberate on care.
  • Verifying patients' insurance coverage and financial capabilities.
  • Developing innovative solutions or suggesting referrals for patients who cannot afford treatment.Qualifications:
    • Bachelor's degree in nursing.
    • Advanced degree in nursing is highly advantageous.
    • Certificate in community and/or hospital case management.
    • Prior experience as an RN case manager in a similar healthcare setting.
    • Driver's license and own vehicle.
    • Familiarity with patient databases.
    • Strong understanding of ethics surrounding patient information.
    • Exceptional leadership, supervision, and motivational skills.
    • Commitment to promoting patients' well-being.
