Talent Healthcare Staffing Inc is seeking a local contract nurse RN Case Management for a local contract nursing job in Mission Hills, California.Job Description & Requirements:

• Duration: 13 weeksPrimary Responsibilities:
  • Conduct intake assessments with patients and families.
  • Ensure families feel equipped and supported.
  • Develop individualized treatment plans.
  • Adjust treatment protocols as needed.
  • Suggest and coordinate follow-up services.
  • Ensure patients have transport to and from homes and healthcare facilities.
  • Meet with healthcare teams to discuss patients' progress.
  • Verify patients have sufficient insurance or finances.
  • Devising innovative solutions or suggesting referrals for patients who cannot afford treatment.Qualifications:
    • Bachelor's degree in nursing.
    • Advanced degree in nursing is advantageous.
    • Certificate in community and/or hospital case management.
    • Prior experience as an RN case manager.
    • Driver's license and vehicle.
    • Familiarity with patient databases.
    • Strong understanding of patient information ethics.
    • Exceptional leadership and motivational skills.
