Our team of healthcare professionals is dedicated to delivering exceptional patient care and exceeding our patients' expectations.Job DescriptionThe Nurse Care Coordinator will play a critical role in ensuring that our patients receive the highest level of care possible. This includes collecting relevant data pertinent to the patients' health or situation, analyzing the assessment data to determine diagnosis and care issues, and developing a plan that prescribes interventions to attain outcomes. The Nurse Care Coordinator will also implement the plan, coordinate care delivery, and employ strategies to promote health and a safe environment.Requirements

• A Bachelor's Degree in Nursing (BSN) is preferred.
• Acute care facility experience is preferred.
• Registered Nurse (RN) licensure in the state of practice is required.
